The principal investigator (PI) of a research project is responsible for communicating, as appropriate, with both the Adult Health Sciences IRB and the HIPAA Privacy Board.

To review any patient Protected Health Information (PHI) (e.g. screening UMKC School of Dentistry (SOD) patients' medical charts for information that will be collected and analyzed for publication) you need to obtain and submit:

Prepare and submit the appropriate form:

• Authorization HIPAA
• Waiver of Authorization form
• Request for Data Access in Preparation for Research
• Data Use Agreement

Note: UMKC IRBs will not release IRB approval documents until appropriate HIPAA applications/approvals have been completed when the research involves certain affiliated institutions, such as the School of Dentistry.
