As part of the Occupational Health & Safety Program, IACUC requires that anyone—who works with or near laboratory animals be assessed for any exposure risk.  This applies to researchers, lab assistants, students and maintenance personnel.

This assessment, or History of Animal Exposure, falls within HIPAA privacy constraints. It is the responsibility of supervisors, principal investigators and ORS managers and administrators to assure that, in addition to themselves, all their staff and students comply with this reporting requirement.

Your report is completed online so that only the medical history reviewer will have access to any medical information that requires review.

Instructions for non-UMKC users

If you are not a member of UMKC and have never logged in to any ORS web service:

1. Select your own username of at least 6 keystrokes, case-insensitive. In case you select a duplicate username, you'll be offered the opportunity to select another one.
2. Select a case-sensitive password following the rules specified below.

Password Rules

• Password length: at least 8 keystrokes (and not more than 120 keystrokes) without any spaces;
• Password must contain at least one lowercase letter (a-z);
• Password must contain at least one uppercase letter (A-Z);
• Password must contain at least one digit (0-9);
• Password cannot be the username, cannot contain any of your names, phone number or address.

Review and Notification

1. Your report is reviewed by a Medical Reviewer.

When your report is complete, the medical reviewer, contracted to UMKC  for this duty, will receive an email that a new report is ready for review. This email will only identify your name. No medical information is transmitted by email.

The medical reviewer will then login with a secure password to access the database with the information you submitted online.

2. If no significant risk is detected, the reviewer’s conclusions are sent to you via email.

Reviews that do not reveal any significant risks associated with working with laboratory animals result in a database entry to that effect. You will receive an automated email describing this conclusion.

3. You may wish to share your report with your supervisor; however you are not obligated to.
4. If a significant risk is detected, the medical review will notify you.

If your information reveals the existence of significant risk associated with you working with animals, you will be contacted by the medical reviewer.

This information, depending on its medical nature, characteristics, circumstances and risks, will be provided to you by one of the following methodologies, solely at the discretion of the medical reviewer:

• Email, detailing any precautions that you should take in working with laboratory animals. The email may instruct you to tell your supervisor of specific instructions or precautions that should be taken (without any disclosure of the medical grounds for such actions).
• Phone call: if additional information is necessary to assess risk.

Other precautionary or preventative actions may be taken by the medical reviewer if deemed necessary based on the information you have provided, fully complying with HIPAA restrictions.

Confidentiality

Regardless of the conclusion of the medical review, the IACUC administrator and others in administratively responsible positions for IACUC and LARC performance and compliance have no access to any of the confidential information you provide in the Animal Exposure report.

Their access is limited to your name, the date that you submitted the report, and the date that it was reviewed.

This access will allow IACUC to demonstrate that you are fully compliant with OSHA Program regulations, while complying with HIPAA restrictions.
